I like to checking on some plastic surgery forums every now and then to get an idea of what men and women are discussing about cosmetic produces, etc.

This question I copied below caught my attention because it isn’t extremely common among young women to have naturally large breasts that cause such discomfort and other health problems.

However my answer to this question would be that, regardless of what age you are, it’s most important that you always feel comfortable in your own body.

Even though this user is only 19 years old in my opinion she should indeed seek breast reduction surgery to alleviate her back pain and concerns of looking disproportional to her body weight.

Dr. Sinclair

Women’s Health Support Forums – Plastic Surgery

Okay so I’m just wanting some honest opinions here.

 So I’m 19, almost 20.
I weigh about 150.
I am a 34 DDD.

I constantly have back aches if I stand to long at a time. And lord knows my back aches after I go on a run.

I am strongly considering getting a breast reduction. I think this will greatly ease my back pains.

I also think that it’ll make me look significantly slimmer. (I’m in college this matters :] )

I don’t really know if this idea is completely out of question because I am so young.

 What are your opinions???

VEXED IN THE CITY SOLVING YOUR WORK-LIFE PROBLEMS

DEAR VEXED: I’m thinking about getting a breast augmentation to help me get ahead at work. Should I? Denise, 26, investment banking

AN author and economist called Catherine Hakim has written widely on an idea called erotic capital. It says that you do better at work and in your life if you are more beautiful. She has a book on this theme to be released soon.

So your question, which initially causes something of a raised eyebrow, is also fair.

But – assuming there is something to Hakim’s main theme – I would first of all question the degree to which erotic capital should be defined by breast size. Big breasts are considered a bonus by some, yes. By others, less so. Regardless, their size is beside the point: what you should be doing in a professional environment is to look as sharp as possible. Wear good clothes, nice shoes, try to be slim and fit.

But being prepared to take to the knife for career advancement seems to me to be a grave skewing of priorities. Being attractive can help your career, perhaps. But on the list of other things that can help your career, it should be ranked fairly low. Intelligence, ambition, hard work, good ethics. Focus on these to get ahead in your career. And attractiveness – particularly breast-size – is a massive double-edged sword. It’s asking for sexual attention to get ahead: something in the same sphere as giving sexual favours to the boss for a promotion. Wouldn’t you rather be promoted because you’re really good? I would.

Dr Hakim seems to me to be playing with fire – she may be stating a truism, that beauty has a positive effect on people’s estimations of you and their desire to help you. But she’s also suggesting that in a world in which women are already relentlessly preoccupied with how they look, they should be even more stressed out about it. Yes, Hakim applies the rule to men, too – but, although they’re getting more worried about looks, they’ve got a long way to go in the skin-deep neurosis stakes. And the male ability to get ahead at work has not been a problem, to date – so being told attractiveness can help won’t carry the same stressful meaning. Set an example by showing that women of all breast sizes can thrive at work. vexed@cityam.com

Reader Question: Mold in Saline Breast Implants?

June 29th, 2011 by admin Leave a reply »

I read online a woman telling about how her saline breast implants got mold and she had terrible problems. Does this happen very often?

A few years ago I put up a YouTube video of my experience with silicone gel breast implants. Now every six months to a year somebody posts a comment about how saline implants are just as dangerous. A frequent portion of that response is a statement about a moldy saline implant. My response is and always has been, if saline implants are so often affected by mold, then why have I never seen it?

I have been implanting (and at times removing) breast implants for over 15 years. You would think if something was a dangerous and common phenomenon that I would be seeing it. I haven’t. Not even once.

A saline implant when left on a table outside of your body can develop mold, but this doesn’seem to happen inside patients. The difference is probably that when implants are properly placed inside a woman’s breasts, her immune system helps protect them from such problems.

Breast implants have risks, but these risks can be minimized by making good choices….choose good surgical talent and facilities, don’t go too large, put your implants under good soft tissue coverage and stay with saline over silicone implants in most cases.

Understand that there are special interest groups of sorts online that want everyone to believe breast implants are dangerous. These groups exaggerate and fabricate on a regular basis to support that belief.

Best Regards,

John Di Saia MD

Breast Revision Options For Previous Breast Augmentation Patients

Breast Revision, a highly specialized and customized procedure, can transform previously unsatisfactory breast implant results in just a short period of time.

According to The American Society of Plastic Surgeons (ASPS), breast augmentation has gone up 22% since 2000 to nearly 300,000 procedures being performed in 2010. And while many of these patients that undergo breast enhancement are quite pleased with their results, there are also patients that may not be. Breast Revision can help both patients who are not happy with their immediate results or who have had breast implants that have aged or changed in the passing years.

Model Photo

According to Denver Breast Augmentation specialist Dr. Gregory Buford, “Prior to surgery, one of the most common complications is unrecognized asymmetry in the breasts. A few years ago, a study was released looking at asymmetry and the findings suggested that a majority of women have some degree of asymmetry. In reality, in clinical practice, I actually feel that almost all women have some degree of asymmetry. This asymmetry must be identified for appropriate preoperative planning to enable the most optimal outcome.”

An experienced and seasoned plastic surgeon who has performed many breast augmentations is sensitive to the nuances and subtle difference in breasts and will adjust their augmentation accordingly to deliver the most symmetric and visually pleasing results.

A common problem that patients who have undergone previous breast enhancement have is one breast may be higher on one side. There could be a number of reasons for this and many ways to correct it, depending on why it appears to be higher.

“Whenever something is implanted into the body, whether it be a hip, joint, or breast implant,” Dr. Buford notes, “A capsule will form around the implant because it is a foreign object.” The body forms a capsule around the foreign object to protect itself from the implant for as long as it is inside the body. While the capsule is soft the majority of the times, there are cases in which the capsule will harden and distort the implant.

“The capsule can tighten down and actually shrink down to distort the implant and patients see is a superior elevation of the implant and feel a very firm breast. At this point, most people think that their implants have became hard,” says Dr. Buford, “However, implants do not become hard and they never do. It is the scar tissue around the implant, the capsule that contracts and distorts the implant, resulting in a overly firm breast.”

This can be solved by removing the implant on the effected side and replacing it with a new implant in addition to making changes to the surrounding capsule. It is recommended that an entirely new implant be used since over time, low-grade bacteria can grow on a breast implant while it is in the body which then cannot be destroyed without the removing the implant.

Another option is surgical revision to the capsule that is formed by the body. In most cases, a simple procedure can be performed so that the breast implant drops and expands to a more natural setting. “Through the initial incision, I can score or add a transverse incision to the capsule to open it up and allow the implant to drop,” states Dr. Buford.

In more complex cases of capsular contracture, Dr. Buford may remove parts of or the entire capsule and reposition the implant within the breast pocket. This is performed depending on the degree of contracture, thickness of breast tissue overlying the capsule and the ease or difficulty of removal associated with capsule revision.

The last option is switching out the type of implant. As technology improves for silicone gel implants, swapping out saline implants for silicone can quickly augment the look and feel of breasts. “9 times out of 10, that actually works very well. For the 1 time out of 10 that it doesn’t work is in patients that have very thin overlying skin or very little glandular tissue to start out with,” Dr. Buford states, “In that case, a very effective way, we’ve found, is to put a sling – an interpositonal graft that thickens the skin between the implant and the breast tissue.”

In most cases of breast revision, the procedure takes approximately an hour and most patients experience as little as 24 hours or less of downtime before they can return to normal activity. The majority of patients can control the pain with over the counter pain medication and may experience some incisional pain but no where near the pain of the original surgery.

Of course, the best way to avoid breast revision is get it done right the first time. Denver breast implants specialist Dr. Buford stresses the importance of seeing an experienced physician who specializes in breast augmentation, “It really comes down to artistry. Patients should look for a surgeon that doesn’t want to just make their breasts bigger, they should look for someone who will customize their particular breast augmentation to them, to their bodies.”

Hello, I’m Dr. Alexander Sinclair, your board-certified plastic surgeon. I was recently reading this interesting article in Wired Magazine and Today, I would like to talk about some exciting developments on the horizon of plastic surgery. You might not know this, but plastic surgeons have been on the forefront of many medical changes. As a matter of fact, the specialty is all about change. Changing people and furthering new developments.

That’s sort of the piece of property that plastic surgeons own. In the 50’s, plastic surgeons were the first doctors to do kidney transplants. Right now, plastic surgery teams are working on stem cell research. What they’re finding is that stem cells occur in fatty tissue. Much more frequently that had been thought.

As a matter of fact, you may find the stem cell in every 200 cells in your adipose or fat cell area compared to one in a few thousand in blood or bone marrow. That means that that is a really right place to find stem cells.

But what does a stem cell do and what is that? We hear about it all the time. Think about this. You have over 200 different types of cell types in your body. For example, you have bat cells and you have bone cells and you have muscle cells and kidney cells. A stem cell is the type of cell that when properly stimulated can develop into any of those cell lines. That’s pretty incredible because if you think about it, one of the real problems with aging or disease or trauma is the loss of certain cells.

For example, if you have a heart attack, you’re going to lose heart cells. Well, there’s no way the body can make new heart cells unless a used to unlock that secret process and stem cells can do that. Now, the applicability here in the plastic surgery world, one of the early applicabilities is for breast reconstruction and even breast augmentation. Because if we can take fat cells and that is the stem cell and then stimulate that stem cell and program it to replicate and build a breast, we’ll be able to build breasts with your own tissue.

That’s an exciting concept and it’s being done. And it’s not just being done in the labs, but it’s being done in experimental settings around the world right now in Europe, Asia, and in the United States. There are programs underway to reconstruct breasts using a person’s own fat by stimulating stem cells. We’re not ready to do this on an everyday basis for breast augmentations, but I would say, within my lifetime it will happen.

So stay tuned. I want to do an update on this because this area is changing rapidly and there are many applications of this stem cell therapy for plastic surgery. I’m Dr. Alexander Sinclair, your board-certified plastic surgeon.

Sassy Australian reality TV show star Tabatha Coffey revealed that a botched breast augmentation almost killed her.

The hair stylist and star of BRAVO’s reality TV show “Tabatha’s Salon Takeover” said her implant ruptured an artery and infected her chest, The Herald Sun reported.

“It was an incredibly traumatic ordeal,” she writes in her upcoming book “It’s Not Really About the Hair.”

“I had lusted after bigger boobs for years, and then they almost killed me,” Coffey said.

Days after the surgery, her left breast moved up under her arm and onto her shoulder.

After extreme pain, she had the implants removed.

“I was better off accepting my real boobs,” she said.

Dr. Michael Fiorillo, a New York City-based plastic surgeon, told FoxNews.com that although Coffey’s case sounds extreme, it is possible.

“An infection can happen for sure. An implant can move into the armpit if you open the pocket too much,” Fiorillo said.

There are different ways a surgeon can put in breast implants. One way is through the underarm, and in that case there is a chance they can move laterally, according to Fiorillo.

“The rupture rate is between one to three percent and it can happen spontaneously or through trauma, such as a car accident,” he said. “The failure rate of the device [implant] is small, but there is a chance they can rupture.”

It is possible that Coffey had saline implants, because they are more likely to rupture than silicone implants.

“I tell my patients, breast augmentation is one of the safest procedures around. The complication rate is not zero, but it is pretty safe. This is a very unusual circumstance,” he said.

My Grandmother Encouraged Me to Get Plastic Surgery

On “Good Morning America” today, 18-year-old Caitlin Clemens shared why she had a breast augmentation to help ease her physical and emotional insecurities. Flashing beneath images of Clemens in a hospital gown was the question “How Young Is Too Young?”

The show also questioned the fact that Clemens’ mom had gone to the same doctor, for the same surgery. Was that right? Of course, there is no right answer to either of these questions, but I can tell you that when I decided to have a breast reduction at age 18 — for many of the same reasons — it was my grandmother who first suggested it, and it saved my life. I was a tall, lanky kid, until my chest sprouted into 34Es. At 5-foot-7, 120 pounds, I was carrying around cantaloupes, each melon the size of my head. They chafed together leaving red welts inside my cleavage, weighed on my petite back and nearly knocked me out during cross-country practice at my small New England boarding school, which had a three-season athletic requirement. “Hey, Gerber,” the basketball team captain yelled out to me. “You have a lot of extra baby food. Can I have some of that milk?” His hyena laughter rang in my ears. I bolted to the nearest women’s bathroom, tears streaming down my cheeks. Despite the sexuality radiating mixed messages from my upper body, I had never taken off my shirt in front a boy, much less myself. I was too disgusted to glance down in the shower. It wasn’t just men who used my chest to deflate me. After practice, surrounded by stick-straight all-American blondes, a female teammate with B-cups turned to me and said, “I think you actually look at lot skinner than you are, because your tits are so big.”

I hid my breasts under sports bras and wrapped them tightly beneath layers of packing tape, preferring a uni-boob to the humiliation of looking down and seeing my cleavage creased together like two pieces of fatty chicken meat. I believed my enormous chest was my fault, and if I worked hard enough I could manage my bra size. At 16, I mistakenly believed my ballooning bust would shrink if I were emaciated. But when I returned to school 20 pounds lighter, after a summer in Israel spent eating oranges and Diet Coke exclusively, I was still suffocating. Although my breasts never changed, something else did: my resolve. The less I ate, I discovered, the more attention I attracted for something other than my boobs. Someone even reported me to the campus therapist. She sat next to me in a long linen dress the color of hay and said, “I’m concerned about your health.” But I didn’t have eating issues; anorexia was never my problem of choice. Then one day, during a three-mile trail race, my legs gave into the pressure. With each pounding step, I had felt shocks of pain rushing up my shins. Now I could feel myself slowing down until I was no longer in motion, watching other runners pass me. I rested on the ground in the woods, unable to stand up, as the last ponytail waved goodbye. The stress of my unhappiness had literally caused cracks in my frail frame, deep fractures in both my legs. As my surgeon father held the shattered black and white images up to the light, he said, “Maybe you weren’t built to be a runner.” To me, his words were a reminder that no matter what I did, I was stuck inside a body I couldn’t stand. At the time, I thought breast reductions were only for old women, like my grandmother, until she pulled me into the bathroom on the first night of Passover. “Let me show you my breasts,” she offered. Without waiting for my response, her lacey bra was draped around her waist, and two perfect C cups pointed at me. “Not one scar,” she bragged. “Lot’s of vitamin E and good skin.” “What did they look like before?” I asked. “Yours.” It was the first time I’d considered surgery. I was desperate to rid myself of the massive inheritance that had weighed on my back and self-image. I was 18 years old, a freshman in college, when I found the courage to ask my father for help. “Please, Dad. I need to get rid of my breasts,” I told him. It was the only time I ever referred to them in front of him. “I’ll set up a consultation as soon as I can,” he said calmly. “I don’t need to consult anyone.” “There are risks — scars, loss of sensation, a 40 percent chance of not being able to breast-feed,” he ticked them off, methodically. “I don’t care. I want surgery,” I blurted. Two months later, I waited behind a white curtain in the surgical wing of Massachusetts General Hospital, fidgeting with my long gown. I shut my eyes and tried to remember a time before my chest consumed me, before I hated my body. I was convinced that all I had to do was this, and I’d be happy. I couldn’t block out the loud hospital sounds — clinking metal instruments and nurse’s sneakers squeaking against the linoleum floors. I jumped when a pretty nurse pulled back the protective curtain. “You’ll be fine,” my mother said as she kissed me on the forehead. My father patted my back silently. I woke up in the recovery room staring at a fuzzy woman. I knew it was my mom because the room smelled like Chanel No. 5. I felt safe again. After four days of pills and sleep, I stood topless in my parents’ bathroom watching my mother remove white gauze, pulling off individual squares crusted with pus and hardened blood. I didn’t recognize the small swollen breasts that fit perfectly inside the cup of my hands. They felt like a burden. There were lines of stitches circling my newly carved nipples and a mixture of brown and purple psychedelic bruises. “Much better,” Mom proclaimed, looking at me in the mirror as if I’d done something important. I knew I had too. She placed her delicate hands on my bare shoulders. “I had no idea you were so thin.” Immediately, my mother saw a change in me. In fact, everyone noticed — everyone but me. I’d put all my chips in the reduction basket, hoping surgery would instantly change the way I saw myself. When it was over, I was supposed to be happy and perfect, but I wasn’t yet. In my mind, I was still top-heavy, no matter what. Now it’s been eight years since my surgery. At 26, I no longer wake up and dread getting dressed. I own bras in colors other than flesh, nude and taupe, and sometimes I don’t even bother with one. The physical scars have faded to the point where I hardly notice them. For the first time, I am consistently running every other day, careful with my legs, which still ache in the center of my shin when I’ve gone too far. A few months ago, I walked into the lobby of my building, sweating after four miles in the sun. While I waited for the elevator, I overheard a petite woman talking with her friend about her new exercise program. “I definitely feel a lot better. Not as good as that girl,” she said, pointing in my direction. When I turned around to a get a look at the great body, I realized no one else was there. “You really didn’t know I was talking about you?” she asked me as we shuffled into the elevator. “I wish I could see whatever it is you just saw,” I confided. “Look again,” she smiled, as the doors opened onto my floor. I haven’t seen the petite woman since that day, but somehow her kind and unexpected compliment has stayed with me, counteracting the tape that had been playing on repeat in my head since puberty. She’d never seen the other me, while I hadn’t allowed myself to see anything else. So, I decided to follow her advice. I pulled out the size-10 Betsey Johnson dress I wore to my high school graduation — the only one I could find that covered me completely — and put it on over my gym clothes in front of the full-length mirror. As the fabric draped off my slender frame, bunching around my small chest, I hardly remembered the insecure person it had once belonged to — the teenage version of myself I’d found the strength to save.

When hundreds of women make use of taxpayer funds for breast enlargement surgery, there should be many more who can benefit too, on medical grounds, suggest surgeons in Australia.

According to the doctors, expanding taxpayer-funded operations to flat-chested women will boost self-esteem.

Medicare claims for breast augmentation have grown by more than 50 per cent during the past five years. Women who had lost their breasts due to diseases such as cancer also fell into the category, but were included in a raft of other items on the Medical Benefits Scheme. The Australian Society of Plastic Surgeons has asked the Federal Government to consider extending the tax-payer-funded benefits to flat-chested women, who suffer what is known in the industry as ‘hypomastia’. Society president Peter Callan said women might feel a “loss of self-esteem due to undeveloped breasts”. “There are a certain number of women with significant hypomastia who would benefit from breast augmentation from both a psychological and physical perspective,” News.com.au quoted him as saying. Breast implants for hypomastia were included under Medicare in the 1970s and 1980s, but the legislation was changed and the item number removed about 10 years ago.

They are commonly used in breast augmentation. More than 300,000 breast augmentations are performed each year in the United States. Silicone is a molecule that contains the elements, silicon, carbon, oxygen, and hydrogen in a repetitive sequence. The sequence of atoms allows the silicone to be made into either a solid, elastomer or gel. Therefore both the shell of the implant and the filling is silicone. The shell is durable and solid it is an elastomer. The filling material is a gel. Breast implants using cartilage, paraffin and other materials have been tried since the 1880’s. But it was not until the 1960’s that the Dow Company with doctors Gerow and Cronin developed a relatively safe product. The first generation implants were tear drop shaped, filled with a thick viscous gel and had a Dacron patch on the back to hold them in place. These implants revolutionized breast augmentation and reconstruction. Surgeons in the 1970’s and 1980’s helped develop the second generation of implants. These implants in an effort to be even more natural had thinner shells and “runnier” gel fills. These were the implants that were most involved in the class action lawsuits of the 1990”s.Third and fourth generation implants have increased safety and performance. These implants have a thicker more cohesive gel and shells with barrier to prevent “gel bleed”. The third and fourth generation implants are those most used today and come in anatomical and round shapes in smooth or textured styles. High cohesive or form stable implants are undergoing testing. This is an abbreviated summary of breast implant history. I have placed thousands of implants over the past twenty five years. I have placed, first, second, third and fourth generation implants, round, and anatomical implants, expander implants, and polyurethane implants. Please allow me to share my knowledge and experience with you. You can schedule a complimentary consultation by calling 310. 274. 4103.

Injecting one’s fat into the breast area to increase size is known as “fat grafting” or “autologous breast augmentation with fat transfer”. Fat grafting is not new, in fact in the 1890’s doctors were using fat grafts to reconstruct breasts. Today fat grafting for cosmetic breast enlargement is controversial. It is not clear how long the fat survives. Doctors should be cautious about injecting fat in women with increased risks for breast cancer. The best advice is to see your plastic surgeon to get information about safe, reliable and long lasting techniques to give you the result you want.

There was no change in muscle flexion (bending), extension or raising the arms when preoperative muscle function was compared with the three-month and six-month evaluation periods. In addition, there was no significant difference in muscle function regardless of implant size.In assessing sensory changes of the breast, both pressure and vibratory testing were performed. Results from the study indicated a significant change in breast sensation at three months postoperatively but the sensation returned by six months, indicating that the change was temporary while the tissue surrounding the breast healed. There was no correlation stated with size of implant or surgical incision.

This is welcome news for all yoga practicing, treadmill running, tennis and golf players. Your performance in the gym shouldn’t suffer and you should be as sensitive as before.

Breast-feeding is known to possess nutritional and immunologic properties superior to those found in infant formulas. Breast-feeding seems to be particularly protective against some of the common childhood conditions such as eczema, otitis media, and iron-deficiency anemia, and is beneficial to neurodevelopment in premature infants. Also, there are substantial health benefits of breast-feeding for the mother and possibly for the whole family. Millions of silicone breast implants have been placed for either reconstructive or cosmetic surgeries. Many of the women with breast implants will become pregnant after their surgery. Women with silicone implants may be apprehensive and confused over whether it is safe to breast feed.

Mothers with breast implants may decide not to breast feed their infants because of the mothers silicone getting into their breast milk. A study by Dr. John Semple published in The Journal of the American Society of Plastic and Reconstructive Surgeons should help them in their decision making. The research shows that when you compare women with breast implants to women who do not have breast implants there is no difference in silicon levels in the breast milk or in blood. Silicon levels in store bought cow’s milk was 15 times higher than in the milk of women with breast implants. Silicon levels in many commercially made infant nutrition formulas was often 80 times higher than the levels in the milk of women with breast implants!

Of course the choice is up to the woman, but this information that women with silicone breast implants have less silicon in their milk than in infant formulas coupled with the health, emotional and developmental benefits of breast feeding makes the choice easier.
